We support the growth of HFW teams across North Carolina.


NC HFWTP is funded by the NC Department of Health and Human Services and operates out of the UNC Greensboro Center for Youth, Family, and Community Partnerships.


What We Do

Our implementation specialists offer high-quality HFW training and credentialing.

We also provide monitoring to assure fidelity in HFW implementations across the state, and technical assistance to communities interested in or already employing HFW care coordination. 


Our Purpose

Mission

The mission of the NC HFW Training Program is to provide training, technical assistance, and coaching to – and advocacy for – communities offering HFW care coordination. 

  • We empower existing local HFW teams as they work with families. 
  • We support the development and expansion of HFW teams across the state, to ensure sustainability of the model now and in the future. 
Vision

The vision of the NC HFW Training Program is for the HFW model to be implemented across North Carolina, so that youth and families feel supported and empowered to overcome challenges, develop resilience, and have access to family-driven, strength-based care coordination.

NC Department of Health & Human Services support

The NC DHHS’ Division of Mental Health, Developmental Disabilities, and Substance Abuse Services contracts with UNCG to support:

  • statewide use of the HFW model
  • the growth of HFW teams

Vroon VDB in Centennial, Colorado provided training, consultation, and oversight during the initial development of the NC High Fidelity Wraparound Training Program. Wraparound Milwaukee and the National Wraparound Initiative provided further consultation and guidance. The Youth and Family Training Institute at the University of Pittsburgh provides ongoing consultation.
