HFW Team Structure

Local HFW teams include:

  • a coach
  • up to 4 wraparound facilitators
  • 3 support partners

COACH

the supervisor, ensures that HFW is implemented with fidelity

  • uses teaching, modeling, and oversight, to prevent drift from the model among the team
  • reviews progress of the youth and family at least monthly
  • ensures agency mandates are met

Facilitators

handle planning, coordination, and monitoring of the wraparound team’s process

  • build rapport and gets to know the family well, to support the family’s vision
  • act in a neutral fashion in terms of coordinating services and team dynamics
  • assist in the development and oversight of the Strengths, Needs, and Cultural Discovery

YOUTH SUPPORT PARTNERS (YSP)

young adults who have had experience working through multiple systems while growing up, and have the ability to engage youth because of their similar lived experiences

  • help youth advocate for themselves
  • help youth navigate different environments
  • model personal interactions and behavior
  • may share their own experiences to build relationships, give youth hope, and motivate them

FAMILY SUPPORT PARTNERS (FSP)

have lived experience with youth moving through various systems

  • support improvement in relationship building between youth and caregiver(s)
  • model effective personal interactions
  • help caregiver(s) advocate for themselves
  • connect the family with resources
